One convenient way to access your health information quickly and easily is through the MyWellSpan portal. This portal allows you to manage your care from any device at any time.
To request a copy of your medical record through MyWellSpan, simply navigate to the Sharing section in the menu and select Request Medical Records. Fill out the necessary information to receive your medical record electronically. If you are unable to use MyWellSpan, you can use a paper form called Authorization to Use or Disclose Health Information instead.
If you need to make changes to your medical record, the best method is through your MyWellSpan account. You can submit a request for a change in your medical record with detailed information by navigating to the Document Center and selecting Requested Records & Record Amendments. Alternatively, you can use an online or paper form such as the Patient Requested Amendment Form or mail/email your request for a change in your medical record.
